One thing is certain from that report. The person who wrote the "Narrative Report of Investigation" is very sloppy with his use of the English language. It is riddled with grammatical errors. A document as important as that should have been reviewed and edited by someone who understands singular and plural concepts and other basic elements of the Mother Tongue.
There are plenty of articles in national news organizations that would benefit from a good editor's review.
